Utah Admin. Code R500-2-4 - Filing and Processing Complaints
(1)
The complainant may file a written, oral, or electronic complaint with the
office no later than three months from the date of the alleged circumstances
giving rise to the complaint. The complainant may file at any of the following:
(a) Multi-Agency State Office Building
Utah Department of Health and Human Services
Office of Ombudsman - 1st Floor
195 North 1950 West
Salt Lake City, Utah 84116;
(b) dhhscustomerexp@utah.gov; or
(c) (801) 538-4580.
(2) The complaint shall include:
(a) a detailed description of the alleged
circumstances which caused the complaint, including dates and
locations;
(b) the names and
contact information of any and all persons involved in those
circumstances;
(c) a detailed
description of any actions taken by the complainant to address the complaint;
and
(d) the desired result or
outcome that the complainant is seeking from the office.
(3) If a complaint does not include the
information in Subsection (2), the office will contact the complainant for more
information.
