W. Va. Code R. § 27-1-9 - Issuance of License
9.1. The
Board shall issue a license authorizing the holder to engage in the practice of
counseling to each successful applicant for licensure as a licensed
professional counselor.
9.2. Each
license issued by the Board shall contain the licensee's name, license number,
the date of issuance and a statement indicating the license expiration date of
June 30.
9.3. The Chairperson and
Secretary of the Board shall sign official licenses, in person or
electronically, and affix with the official seal of the Board.
9.4. All licenses issued by the Board remain
the property of the Board and the licensee shall surrender his or her license
upon demand.
9.5. Once all
requirements for licensure have been determined, the Board shall send
notification of the license to the applicant's last known email
address.
