W. Va. Code R. § 64-21-4 - Inspections
4.1. The health
officer shall inspect each child care center at least once each six (6) months.
The health officer shall also make additional inspections needed to determine
satisfactory compliance with the provisions of this rule and any orders,
notices, instructions, or specifications issued pursuant to this
rule.
4.2. The health officer may
consider any consecutive violation of the same item or items of this rule as
cause for the immediate suspension of a permit to operate.
4.3. The operator shall post a copy of the
inspection report within the child care center.
4.4. The owner, operator or person in charge
of a child care center, shall provide the health officer with free access to
the entire premises for the purpose of inspection, and shall furnish all
requested information to make the inspection complete.
