Wis. Admin. Code Department of Health Services § DHS 154.035 - Termination of eligibility
Eligibility for the adult cystic fibrosis program is terminated if either of the following events occur:
(1) The patient dies.
(2) The patient moves out of the state of
Wisconsin.
