044-63 Wyo. Code R. §§ 63-16 - Disclosure Requirements
(a) Each insurance carrier shall include a
description of the external review procedures in or attached to the policy,
certificate, membership booklet, outline of coverage or other evidence of
coverage it provides to claimants.
(b) The description required under subsection
(a) shall include a statement that informs the claimant of the right of the
claimant to file a request for an internal or external review in compliance
with W.S. §
26-40-201.
(c) In addition to subsection (b), the
statement shall inform the claimant that, when filing a request for an external
review, the claimant will be required to authorize the release of any medical
records of the claimant that may be required to be reviewed for the purpose of
reaching a decision on the external review.
