206-1 Wyo. Code R. §§ 1-5 - Application Procedure

Application deadlines and procedures shall be widely publicized to facilitate equal access to the funds on the part of all potential grantees.

(a) Applications shall be on forms prescribed by the Board, and received by such date(s) as the Board shall establish.
(b) The application shall include the following:
(1) Name and address of applicant(s);
(2) Description of need for funds;
(3) The manner in which the need will be met;
(4) The dollar amount requested;
(5) Number of persons who will be served;
(6) Whether the costs will be one time or ongoing; and if ongoing, how the program will be funded in the future;
(7) How the proposed grant activities can be expected to lessen the occurrence and reoccurrence of child abuse and neglect;
(8) A plan to reduce the need for government intervention in child abuse and neglect prevention and education, which must be clearly stated in terms of measurable results;
(9) Schedule for project completion of each activity;
(10) A description of the applicant's intended evaluation and accountability procedures;
(11) Three letters of support from local community organizations or agencies;
(12) Certification that the applicant's request does not duplicate or supplant funds available from other sources; and
(13) Any additional information the Board shall require.
