048-2 Wyo. Code R. §§ 2-5 - [Effective until 9/26/2024] Renewal of Certification
(a) A renewal certification grants the same
rights and imposes the same duties as an initial certification under Section
4(b) of this
Chapter.
(b) In order to renew
certification, a certified provider shall submit to the Department a complete
renewal application in the form established by the Department through the
Department's public website.
(i) A renewal
application must:
(A) Be submitted to the
Department no less than thirty (30) calendar days prior to the expiration date
of a provider's certification; and
(B) Provide documentation or other evidence
that the provider continues to satisfy the certification standards established
under Section
8 of this Chapter or continues to maintain
national accreditation.
(c) Upon receipt of a complete application to
renew certification, the Department shall review the application for compliance
with these Rules.
(i) The Department's review
may include an on-site inspection and independent verification of national
accreditation with the accrediting body, if applicable.
(ii) The Department shall approve or deny an
application within thirty (30) calendar days after receiving the
application.
