Ariz. Admin. Code § R3-11-201 - Application for a Veterinary Medical License

A.An applicant for a veterinary medical license shall submit an application packet to the Board that contains:
1. A notarized application form signed by the applicant that contains the information set forth in A.R.S. § 32-2213;
2. The documents required under R3-11-203; and
3. The applicable fees, payable by certified check or money order:
a. If applying for a regular license, the applicant shall submit the application fee required in R3-11-105.
b. If applying for a license by endorsement under A.R.S. § 32-2215(C) or a specialty license under A.R.S. § 32-2215(D), the applicant shall submit the application and license issuance fees required under R3-11-105.

C. If an applicant has passed the North American Veterinary Licensing Examination and is required to take only the state examination, the applicant shall submit the application packet required under subsection (A) no later than 30 days before the date the applicant intends to take the state examination.

D. C.If an applicant is required to take the North American Veterinary Licensing Examination , the applicant shall apply directly to the National Board of Veterinary Medical Examiners.
