Ariz. Admin. Code § R4-28-1305 - Notice of Appearance of Counsel
A. A party may participate in the party's own
behalf or be represented by a member of the State Bar of Arizona.
B. Any person intending to appear at a
contested case hearing or appealable agency action as counsel or representative
of a party shall file a Notice of Appearance which shall advise the Department
of the person's intent to appear on behalf of a party. The notice shall be
filed with the Office of Administrative Hearings and served on all parties and
shall contain:
1. The title of the
case,
2. The name of the agency
ordering the hearing,
3. The
current address and telephone number of the person appearing, and
4. The name of the party for whom the person
is appearing.
