Ariz. Admin. Code § R9-10-310 - Transport; Transfer
A. Except as
provided in subsection (B), an administrator shall ensure that:
1. A personnel member coordinates the
transport and the services provided to the patient;
2. According to policies and procedures:
a. An evaluation of the patient is conducted
before and after the transport,
b.
Information from the patient's medical record is provided to a receiving health
care institution,
c. A personnel
member explains risks and benefits of the transport to the patient or the
patient's representative, and
d. A
personnel member communicates or documents why the personnel member did not
communicate with an individual at a receiving health care institution;
and
3. The patient's
medical record includes documentation of:
a.
Communication or lack of communication with an individual at a receiving health
care institution;
b. The date and
time of the transport;
c. The mode
of transportation; and
d. If
applicable, the name of the personnel member accompanying the patient during a
transport.
B.
Subsection (A) does not apply to:
1.
Transportation to a location other than a licensed health care
institution,
2. Transportation
provided for a patient by the patient or the patient's
representative,
3. Transportation
provided by an outside entity that was arranged for a patient by the patient or
the patient's representative, or
4.
A transport to another licensed health care institution in an
emergency.

from the patient's medical record, including orders that are in effect at the
time of the transfer, is provided to a receiving health care institution;
and
c. A personnel member explains
risks and benefits of the transfer to the patient or the patient's
representative; and
3.
Documentation in the patient's medical record includes:
a. Communication with an individual at a
receiving health care institution;
b. The date and time of the
transfer;
c. The mode of
transportation; and
d. If
applicable, the name of the personnel member accompanying the patient during a
transfer.
