12 CCR 2512-2-12.120.3 - CLIENT NOTICE OF RIGHT TO A FAIR HEARING

A. any recipient shall be mailed a notice whose EBT account is to be adjusted by the EBT contractor, advising the recipient of the right to a fair hearing.

B. The notice shall stipulate:

1. The amount to be adjusted;

2. The name of the retailer;

3. The reason for the adjustment;

4. The date of the adjustment;

5. Procedures for requesting a state hearing; and,

6. The timeframe for requesting a hearing.
