Fla. Admin. Code Ann. R. 64B3-11.004 - Procedures for Approval of Provider Programs
The provider seeking approval:
(1) Shall apply to the Department using Form
DH 1052, (7/97), incorporated by reference herein and available by request to
the Board, and submit the application fee set forth in Rule
64B3-9.001, F.A.C., prior to the
first course being offered.
(2)
Shall be granted initial approval for the biennium in which the application is
submitted.
(3) Shall be subject to
periodic review. Approval may be withdrawn if the Board determines that
adherence to standards outlined in Rule Chapter 64B3-11, F.A.C., is not
maintained or if information submitted to the Board by the provider is found to
be a material misrepresentation of fact.
(4) Shall use the program approval numbers,
if applicable.
(5) Shall be granted
authority to give continuing education courses without additional Board
approval once they are offered by a program with approval status.
(6) Shall demonstrate continued compliance
with the requirements of Rules
64B3-11.002 and
64B3-11.003, F.A.C., through
periodic review and random audits of continuing education offerings. Audits
shall be conducted for cause and randomly during renewal of the continuing
education program.
