31 IAC 5-9-1 - Short term disability; eligibility

Sec. 1.
An employee who meets the following criteria is entitled to short term disability benefits under this rule:
(1) The employee is employed on a full-time
basis and has worked at least six (6) consecutive months of continuous
full-time employment while actively working without a break in service due to:
(A) resignation;
(B) retirement;
(C) a layoff;
(D) an unauthorized leave of absence;
or
(E) termination; before the
elimination period begins. An authorized leave of absence without pay, while
not considered a break in service, is not counted toward the six (6) months of
active work requirement, so that the length of authorized leave without pay is
added to the period before the employee becomes eligible.
(2) The employee is absent due to illness,
injury, or legal quarantine as evidenced by a physician's signed
statement.
(3) The employee is
absent thirty (30) consecutive calendar days due to the disability.
