Kan. Admin. Regs. § 4-13-10 - Application for governmental agency registration
An application for a governmental agency registration or for renewal of a governmental agency registration shall provide the following information in addition to that required by K.S.A. 2-2440(e):
(1) If the applicant is a township, the
county in which said township is located.
(2) Names of certified applicators and
uncertified persons working under their supervision who will work under the
governmental agency registration being applied for.
(3) Signature and title of the official
submitting the application.
(4)
Date the application is submitted.
