N.M. Code R. § 9.2.23.13 - FORMS
A. The department
shall prescribe forms for implementing the Patient Care Monitoring Act and this
rule. No facility shall use any forms other than those prescribed by the
department.
B. A facility shall
maintain the original copies of all completed forms relating to a patient for
at least three years from the date of the patient's discharge from the
facility. The forms shall be accessible to the program at all times.
