Wash. Admin. Code § 381-100-160 - Acknowledgment of rights
The community corrections officer shall forward to the board and attorney general signed copies of the receipt and acknowledgment of these rights along with copies of the factual allegations. Should the offender refuse to sign either the factual allegations or notification of rights, the community corrections officer shall witness the refusal and note the time and place of service.
