Or. Admin. R. 851-053-0005 - Application for Initial Licensure to Practice as an APRN in Oregon including those licensed in another state or country

(1) An applicant for APRN licensure must:
(a) Submit a completed application as defined in OAR 851-006-0030(17).
(b) Hold an unencumbered registered nurse license in the State of Oregon. The RN and APRN application may be completed simultaneously, however, the RN license must be issued prior to issuing an APRN license.
(c) Abide by all licensing requirements as described on the application form.
(d) Provide primary source documentation for the following educational requirements:
(A) A Master's Degree in Nursing, or if the APRN preparation program did not offer a Master's in Nursing, a Master's Degree congruent with the licensure being applied for, or a Doctorate in Nursing. Any applicable degree in nursing must be from a program accredited by a national nursing organization recognized by the US Department of Education except for the following:
(i) For Nurse Practitioners: A Master's Degree is not required for applicants who graduated from their program prior to January 1, 1986.
(ii) For Certified Registered Nurse Anesthetists: A Master's Degree is not required for applicants who graduated from their program prior to December 31, 2000.
(iii) For Clinical Nurse Specialist a Master's degree is required regardless of graduation date.
(B) For graduates of programs outside the U.S., a credentials evaluation from a Board approved credentials service that demonstrates educational equivalency to U.S. Master's in Nursing degree in alignment with OAR 851-053-0005(1)(d)(A).
(e) Have primary source evidence of advanced level course content reflecting the concepts in advanced health assessment, advanced physiology and pathophysiology, and advanced pharmacology.
(f) Applicants who graduated from an APRN program prior to the dates as noted in OAR 851-051-0020(3) will be considered for licensure without providing evidence of course content reflecting advanced pharmacology, advanced physical assessment, or advanced physiology, if:
(A) The applicant has been continuously licensed in a jurisdiction of the U.S. since initial program completion; and
(B) Has no practice discipline placed on their license since being initially awarded a license to practice in a jurisdiction of the U.S.
(g) Provide primary source verification of national certification from a national certifying body that meets the requirements of OAR 851-053-0035 that is congruent with the population foci being sought based upon the following dates:
(A) For Nurse Practitioners: As of January 1, 2011, all Oregon applicants must provide primary source evidence of national certification.
(B) For Certified Nurse Anesthetists: Primary source verification of national certification is required for all initial Oregon license applications.
(C) For Clinical Nurse Specialists: National Certification is not required for licensure.
(h) Meet practice requirements by:
(A) Applicant has 400 hours of APRN practice within their population foci within the two years preceding submission of a completed application, or;
(B) Graduated from their qualifying APRN program within the last two years, or;
(C) If it has been less than two years since completion of a Board approved re-entry program, or;
(D) Applicant has applied for re-entry per OAR 851-053-0030.
(2) Per ORS 678.050, the Board has the final authority to determine acceptable documentation of licensure requirements.
(3) Per ORS 678.111 the Board has the authority to deny license application for specific findings during the application process or if the requirements for licensure are not met. Application fees are not refundable if requirements are not met, or application is denied.
(4) The Board retains the authority to conduct a random audit of licensees. Upon request of the Board, licensee shall submit documentation of compliance.
